What evidence supports the efficacy and safety of estriol-based facial creams?
Estriol in facial creams actually has a solid scientific foundation behind it, both for how well it works and how safe it is. Here at Alloy, we leaned heavily on this research when developing the M4 skincare line, and then we took it a step further by running our own independent clinical studies.
What shows it works
Several published studies have looked at topical estrogens for skin aging. They consistently show that estrogen loss in perimenopause and menopause leads to thinner, drier, less elastic skin with faster collagen decline. When estriol is applied directly to the skin, it stimulates collagen, increases hydration, and improves elasticity right at the site of application.
The independent clinical study on our prescription M4 Face Cream Rx found strong results in just 12 weeks, including large improvements in elasticity, hydration, texture, and overall skin health. You can see the full report if you'd like the details: M4 Face Cream clinical study.
We also studied our Eye Cream Rx, which uses a higher concentration of estriol. Women saw measurable improvements in firmness, wrinkle reduction, and eyelid crepiness. Those results are published too: Eye Cream clinical study.
If you want a quick explanation of the historical research on estriol creams, Dr. Sharon Malone talks about why estriol was chosen and how much data supports it in this YouTube segment:Science behind estriol creams.
What shows it’s safe
Across both the scientific literature and Alloy’s own studies, topical estriol doesn’t raise systemic estrogen levels. That’s an important point because it means estriol stays in the skin rather than entering the bloodstream. There were no hormonal side effects in any of the research referenced on our site, and our clinical trials measured blood levels to confirm they remained unchanged.
You can also see this echoed in the broader dermatology literature. The 1996 International Journal of Dermatology study linked on our site looked at topical estrogen for facial skin and found no increase in serum hormone levels. Another study linked on our blog also confirmed the same outcome.

So the short answer is yes, there is strong evidence for both the effectiveness and the safety of estriol-based facial creams, and the data just keeps getting better as more studies are completed.
