Clinical guidelines, educational materials, and insurance coding systems will all need to be updated, so long story short, it’s going to be a minute until this takes place. Until then, you'll likely see both names used interchangeably, but the shift reflects something experts have known for a long time. The name is just finally catching up to match the science.

Why This Changes How We Think About the Condition

For years, conversations about PCOS were largely centered around fertility. Once a woman was finished having children, or never wanted them in the first place, the conversation often faded too. The PMOS framework recognizes that reproductive health is only one piece of a much bigger picture.

Insulin resistance, chronic inflammation, abnormal lipids, and an increased cardiovascular risk don't just disappear after your reproductive years. The truth is, these effects can span far into midlife and the years following and worsen over time without intervention. 

Conversations with your clinician may start focusing less on your menstrual cycle and more on your blood pressure, cholesterol level, blood sugars, liver health, body composition, and other metabolic markers.

How Perimenopause Amplifies PMOS Symptoms and Health Effects

Just when you think you've got PMOS management down, your hormones throw in another plot twist: perimenopause. This may mean that, despite sticking to your tried-and-true healthy, low-carb diet, you suddenly start noticing weight gain, especially around your midsection.

The policy brief also encourages employers and healthcare systems to think more broadly about supporting women during midlife as they enter into perimenopause. This may include greater flexibility for medical appointments, workplace environments that better accommodate symptoms like heat intolerance, and improved access to tools that help women monitor their metabolic health when appropriate. 

Managing PMOS During Midlife

The PMOS rebrand isn't changing how the condition is treated overnight, but it is changing how many clinicians think about long-term treatment.

Lifestyle interventions remain foundational, but the focus may shift during midlife. Rather than trying to out-exercise changing hormones with more and more cardio, many experts recommend making strength training a bigger priority. Muscle plays a major role in glucose (sugar) metabolism, so building and maintaining lean muscle mass can help improve your insulin sensitivity while also supporting bone health and healthy aging.

Nutrition is also so important! For some women, medications that improve insulin sensitivity, such as metformin, continue to play a central role in treatment. Others may benefit from an inositol supplement or even GLP-1 Agonist medications.

A continuous glucose monitor (CGM) may also help provide some helpful data points for some people. Not everyone will need a CGM, but it can provide insight into how your body responds to different foods, exercise, stress, and even sleep, making it easier to identify patterns that might otherwise go unnoticed.
