How should common GLP-1 gastrointestinal side effects be managed?

At Alloy, we always want you to feel supported, not sick. Mild gastrointestinal side effects are common with GLP-1 medications, but they should be manageable and improve as your body adjusts.

Here’s how we recommend managing them:

1. Start low and titrate slowly

Side effects are often strongest after the first dose at a new level. Many patients find that symptoms improve over the next few weeks at the same dose. That is why, here at Alloy, your doctor tailors dosing month by month instead of automatically increasing it. If you are losing weight steadily or feeling unwell, we may hold or even lower your dose.

If symptoms are disruptive, message your Alloy doctor. In some cases, we can adjust your dose or prescribe anti nausea medication.

2. Eat smaller, protein focused meals

GLP-1s slow stomach emptying, so large meals can worsen nausea or discomfort.

  • Try smaller, more frequent meals

  • Aim for 75 to 100 grams of protein daily

  • Avoid high fat, greasy, spicy, or sugary foods

  • Skip carbonated drinks if they worsen bloating

3. Prioritize fiber and hydration

Constipation is common because digestion slows and appetite decreases.

  • Drink at least 64 oz of water daily

  • Make sure you are getting enough fiber from whole foods

  • Consider a fiber supplement if needed

4. Be mindful of food choices early on

When starting a GLP-1, avoid experimenting with rich or unfamiliar foods. Your stomach is emptying more slowly than it ever has before, so heavy meals can trigger nausea.

5. Know what is not normal

Mild nausea, constipation, loose stools, or reflux can happen. However:

  • Vomiting is not how these medications cause weight loss

  • Persistent or severe vomiting is a red flag

  • Feeling so unwell that you cannot function is not expected
