How does topical use of estriol differ from systemic hormone therapy for skin concerns?

Topical estriol and systemic hormone therapy can both support healthier skin in midlife, but they work in really different ways.

Topical estriol, like what we use in our M4 skincare line, stays local on the skin. Dr. Ellen Gendler and Dr. Corinne Menn both explain that when estriol is applied directly to the face, it binds to estrogen receptors right where collagen, elastin, and hydration have declined. That means you get improvements in firmness, moisture, and texture without raising estrogen levels in your bloodstream. Studies, including our own independent clinical research, show that topical estriol can thicken collagen, smooth fine lines, and restore hydration while keeping systemic absorption almost undetectable. It’s more like giving your skin a targeted treatment right where it needs it.

Systemic hormone therapy works very differently. It circulates throughout the body and can provide whole body benefits, including skin, though the skin improvements are more of a secondary effect. In the chapter on Estrogen Therapy for Skin, Dr. Menn explains that systemic estrogen helps support skin health but doesn’t focus on the face specifically, and it takes sustained use to reach those surface-level receptors.

She describes it kind of like the difference between taking an oral steroid versus using a gentle steroid cream on a rash. One works broadly throughout the whole body, and the other treats exactly the area you want.

Because of those differences, many women who take systemic hormone therapy still get added benefit from using topical estriol on their face. And for women who can’t or choose not to use systemic therapy, topical estriol gives you a way to directly address the skin changes that come with declining estrogen.

If you want to hear a quick explanation of how topical estriol works on the skin, Dr. Deepti Gandhi covers it nicely in this short chapter of her video: Topical Estriol for Skin.
